What is in the template

  • Case: 8D number, customer and part, the problem in one line, the date the team was formed and the customer due dates.
  • Decision rights: Who the champion is and what they approve (resources, priority, permanent actions), and what the team leader owns.
  • Team members: Role, name, function, why the person is on the team, which D-steps they work on and roughly how much time per week.
  • Working rules: Meeting rhythm, where actions are tracked, who updates the customer and how a due date at risk is escalated.
  • Team changes log: Who joined or left, when and why, so a reviewer can follow the team through the 8D.

The last page is a filled example for a fictional coating defect on a bracket. All companies, people and data in it are invented; replace everything with your own facts.

Why a written D1 can help

Some customer 8D forms only ask for a list of names in D1. A short charter can make it easier to show that the people who know the process are on the team, that someone can approve the permanent action, and that the team changed when the investigation moved. It is a working tool for your team; your customer's 8D form and requirements come first.

Quick checks before D2

  • Every function that touches the failure is named: where it was made, where it could have been detected, and where it was shipped from.
  • The team leader is not the only person doing the analysis.
  • The champion can actually approve the permanent action and the resources it needs.
  • Customer or supplier people are added when the investigation points to them, with the reason noted.

For roles, champion vs team leader and team size, see the D1 team formation guide. Next comes the D2 problem description.
