What is in the template

  • Identification: NCR number, who raised it and where, part, revision, lot and the source (incoming, in-process, final, customer or audit).
  • Requirement vs found: The requirement with its document and revision next to what was actually found, the quantities and the evidence kept.
  • Containment: Each place suspect product could be (line, finished stock, in transit, at the customer, other lots of the same component) with quantities and where it was moved.
  • Disposition: Use as is with concession, rework, repair, scrap, return to supplier or other, with who approved it and the re-inspection result.
  • Cause and corrective action decision: The direct cause known so far and whether an 8D, CAPA or SCAR is needed, with the reason.
  • Cost and closure: Optional cost lines, and a closure check that every nonconforming part has a disposition.

The fields are a generic set. If your procedure or your customer prescribes an NCR or reject form, use theirs.

The filled example

Pages 1–2 are the blank form. Page 3 shows a fictional NCR on the same housing with a pressed bushing as the control plan template example: a start-of-shift push-out test fails, the parts from the suspect bushing lot are held on the line and in finished stock, and no non-destructive check is available in this example, so all 1680 are scrapped. On a real NCR, sorting is considered first, and your customer or your controlling procedure decides the disposition; scrapping is not the default. The NCR then opens SCAR-EX-0041, an information request to the bushing supplier for lot data, with a read-across in the PFMEA. All companies, people and data are invented.

NCR, 8D or CAPA?

An NCR records one nonconformance and what happens to the affected product. It answers "what do we do with these parts?". An 8D or CAPA answers "why did it happen and how do we stop it coming back?". An NCR can close with a disposition only, for example a one-off damage; if the cause can repeat, a corrective action is opened and its number is written on the form. If the customer reported the problem, follow the customer's 8D or SCAR requirements.

Quick checks before you close an NCR

  • Every place suspect product could be was checked, including stock and anything in transit, parts made since the last good check, and other lots of the same component.
  • The quantities add up: checked, nonconforming, reworked, scrapped.
  • "Use as is" or a repair has the approval your procedure and your customer require, in writing.
  • Reworked parts were re-inspected and the result is recorded.
  • Rework is the correction for these parts; if the cause can repeat, a corrective action number is written on the NCR.
